This is a difficult one to describe so probably best to watch the screencam video below and try to replicate the problem for yourself.

Basically if you have a family with existing children and you have another child unattached in your file then presently select the parents and use the old RM option of Add > Child.

If you select the child, use the new onscreen + Click to add father, select existing person and then add the mother in the same fashion you will notice that both the father and mother both show as having two spouses. One set of spouses will show the child you just entered and the other will show the children that were already with the partnership. If you check your database you will find that you only have one set of (parent) individuals.

This has still to be resolved in version 4.0.6.
